In 2019, the FDA issued a formal safety communication acknowledging breast implant illness (BII) as a recognised set of symptoms that some patients attribute to their breast implants. It is real enough that the FDA acted on it. It is not well enough understood that anyone has a complete explanation for why it occurs or who will develop it.
If you are considering breast implants in Delhi NCR, the information below is not designed to discourage you. It is designed to ensure the conversation you have with your surgeon before the procedure is more complete than most patients are offered.
What Is Breast Implant Illness?
Breast implant illness is an umbrella term for a range of systemic symptoms that some patients develop after breast augmentation in Delhi NCR that they attribute to their implants. The symptoms are not localised to the breast. They are systemic: persistent fatigue, cognitive difficulty (described by patients as brain fog), joint pain, hair loss, skin rashes, and immune irregularities.
There is no single diagnostic test for BII. There is no published biological mechanism that fully explains it. The evidence that it is implant-related comes primarily from case series and patient registries showing that many affected patients experience symptom resolution after explantation. That is a meaningful signal, even without a confirmed mechanism.
Published estimates on the prevalence of BII vary widely. An estimated 1 to 10 per cent of breast implant patients report symptom clusters consistent with BII. The wide range reflects the absence of a standardised diagnostic framework.

Is Breast Implant Illness the Same as BIA-ALCL?
No. These are two distinct conditions and conflating them causes unnecessary alarm in both directions.
BIA-ALCL (Breast Implant-Associated Anaplastic Large Cell Lymphoma) is a rare cancer of the immune system, not the breast. It has been causally linked to textured implants, specifically those with a macro-textured surface. The rough surface appears to create a chronic inflammatory environment that, in rare susceptible individuals, promotes the lymphoma. Smooth implants have not been causally linked to BIA-ALCL in published evidence.

BII, by contrast, occurs across implant surface types, affects some patients with smooth implants, and may have a different mechanism involving systemic immune sensitisation or silicone particle micro-exposure. They share the word ‘implant’ in their names, but clinically and biologically, they are separate concerns.
Who Is at Higher Risk of Breast Implant Illness?
The published literature identifies several factors associated with higher BII symptom reporting: pre-existing autoimmune conditions, a personal or family history of autoimmune disease, connective tissue disorders, and a genetic predisposition to heightened immune sensitivity. A 2021 systematic review in Plastic and Reconstructive Surgery found that patients with pre-existing autoimmune diagnoses had higher rates of symptom attribution to implants following augmentation.
This does not mean that patients with autoimmune conditions cannot have breast implants. It means the risk-benefit discussion at the pre-operative consultation must be more thorough for these patients, and the monitoring plan after surgery must be more active.
What Should Indian Patients Specifically Ask Before Getting Breast Implants in Delhi NCR?
- Are you using smooth implants? Smooth cohesive silicone implants are not causally linked to BIA-ALCL. Confirm this before the procedure date.
- What is the monitoring programme? The FDA recommends MRI from three years after placement and every two years thereafter for silicone implants. Confirm this is part of the post-operative plan.
- Do you have a pre-existing autoimmune or connective tissue condition? If yes, this must be disclosed to the surgeon before the procedure, not after symptoms develop.
- What is the explantation plan if symptoms develop? A surgeon who provides breast implants should be able to perform or refer for explantation. Confirm this upfront.
- Is the implant CDSCO-approved? Only implants approved by India’s Central Drugs Standard Control Organisation are regulatory-cleared for use in India. Ask for the specific implant brand and approval status.

What Happens if You Develop Symptoms After Breast Implants?
Document the symptoms and their timeline relative to your implant date. Present to the surgeon who placed the implants and request a full clinical assessment. Request ultrasound or MRI to rule out implant rupture, BIA-ALCL, and other structural causes of the symptoms before BII is concluded as the explanation.
If BII is suspected and no other structural cause is found, explantation with total capsulectomy (removal of the implant and the surrounding scar capsule) is the definitive intervention. Published case series report symptom improvement in 70 to 80 per cent of BII patients after total capsulectomy with explantation. Re-augmentation after symptom resolution is possible in appropriate patients following a supervised observation period.
Indian patients can report adverse events related to breast implants to CDSCO (Central Drugs Standard Control Organisation) via the Pharmacovigilance Programme of India.
The Balanced Reality
The majority of the millions of patients who have had breast implants globally do not develop BII, BIA-ALCL, or significant complications. Published long-term safety data on smooth cohesive silicone implants show high satisfaction rates and manageable complication profiles. The appropriate response to BII is informed consent and active monitoring, not avoidance of a procedure that benefits the majority of patients who choose it.
